FEE POLICY

The African American Lupus Foundation, Inc. will provide Case Management Services for Jobs, Employment, Training Programs or referrals for a one time fee of 25% of the first pay check upon the successful completion of a work agreement for client. The 25% fee includes assistance with resume writing, copying, printing, faxing, phone calls to potential employer and client.

The fee is payable and due upon the receipt of the first pay check by the client or within 5 days of the receipt of the first pay check. Failure to pay the fee within the first 5 days will result in a referral to a collection agency or complaint to be filed in a local court of law. All collection fees will be added to the 25% first pay check fee.

Being of sound mind and body, I______________________have read and understand the above fee policy and I agree to pay the

African American Lupus Foundation Inc., 25% of my first pay check upon the day that I receive the first pay check or within 5 days of receipt of the first pay check. I agree to pay all court cost and collection fees if I fail to make the fee payment upon the day that I receive the first pay check or within the 5 days of receipt of my first pay check.
